If you do not sleep under a sheet or blanket, it is probably reasonable to insert it so it stays in place of tossing and turning a full night's sleep. If you go to sleep under, then folding is an incredible inconvenience.
Pillows with Anime characters seem to be pretty normal in Japan. Well, pretty normal for guys who attend conventions and stuff anyway.
